Left ventricular noncompaction (LVNC) is a rare congenital condition defined by the presence of prominent trabeculations in myocardial layer left ventricle. The clinical presentation varied as some patients are asymptomatic and others have symptoms decompensated heart failure, arrhythmias, or thromboembolism. We present case 42-year-old female with past medical history asthma substance use disorder who presented to Emergency Department following syncopal event. patient had used heroin intranasally, which she became unresponsive for several minutes. Her husband witnessed event initiated chest compressions. When examined emergency services (EMS), palpable pulse was given naloxone. underwent further evaluation admitted treatment aspiration pneumonia. Throughout her hospital stay, complained pain musculoskeletal characteristics, likely secondary However, due persistence pain, cardiac done. electrocardiography (EKG) revealed normal sinus rhythm no acute ischemic changes. echocardiography apical systolic diastolic function, line diagnosis LVNC. Upon discharge, extensively counseled abstain from follow up cardiology monitor. Given initial high-risk behavior, would benefit close follow-up arrhythmias.
